Assistante Technique: Prise en Charge a Domicile (PECADOM)

Le Mali a bénéficié des  ressources de la Série 6 et 10 du Fonds mondial pour le programme Paludisme. Grâce à cet apport du Fonds mondial, et des autres partenaires au développement du Mali, la lutte contre le paludisme connaît un regain d’intérêt avec la mise en place des stratégies novatrices et efficaces telles que : l’utilisation desCombinaisons Thérapeutiques Antipaludéennes à base d'Artémisinine (CTA) en remplacement de la chloroquine, la Pulvérisation Intra domiciliaire (PID), la distribution de masse de Moustiquaire Imprégnées d’Insecticides a Longue Durée (MILD) et la protection des femmes enceintes par le Traitement Préventif Intermittent (TPI) basée sur la Sulfadoxine Pyriméthamine (SP). Pour cette subvention, la prise en charge effective sera renforcée par une approche communautaire intégrée comportant l’utilisation des Tests de Diagnostic Rapide(TDR) et le traitement précoce par les CTA. Toutes ces interventions seront soutenues par une mobilisation sociale et un système de suivi et évaluation.

Au Mali, il existe plusieurs faiblesses dans le système communautaire et qui affectent les résultats en matière de lutte contre le paludisme.  Il s’agit de l’absence de politique et de directives pour la prise en charge au niveau des ménages, l’insuffisance de ressources humainesà tous les niveaux pour la prise en charge, l’insuffisance de supervision des Relais Communautaires;la faible responsabilisation des communautés dans les activités de lutte contre le paludisme ; la faible capacité des organisations de la société civile (ONG et Associations) dans l’organisation locale des communautés à agir et dans l’appui à la prise en charge des cas de paludisme ;le recours tardif des populations aux soins de santé à cause des croyances et attitudes de la population, l’insuffisance dans la référence/évacuation des cas de paludisme grave du village au Centre de Santé Communautaire(CSCom) et l’irrégularité et l’insuffisance des activités de communication au niveau communautaire parmi d’autres.

Pour adresser ces faiblesses à travers la subvention en cours, PSI/Mali recrute, au profit du Programme National de Lutte contre le Paludisme (PNLP), un Conseiller Technique Prise en Charge A Domicile (PECADOM) pour l’assister dans la mise en œuvre de la subvention consolidée Paludisme du Fonds mondial pour une durée de 45 jours. Le conseiller technique reportera à la Direction du PNLP et sera basé dans les locaux du PNLP.

RESPONSIBILITES:

  • Concevoir et proposerles documents cadres et les orientations des activités du PNLP au niveau communautaire ;
  • Conseiller la Direction du PNLP et les instances du Ministère de la Santé dans le cadre des soins essentiels au niveau communautaire;
  • Proposer des mesures organisationnelles et techniques susceptibles d’améliorer la prise en charge du paludisme, le suivi-évaluation, le rapportage  et la coordination entre les partenaires du programme au niveau communautaire ;
  • Renforcer les capacités du système de santé à répondre à la demande communautaire dans le cadre de la PECADOM;


RESULTATS ATTENDUS

  • Les documents cadres et les orientations stratégiques de la PECADOM en rapport avec la politique des Soins Essentiels dans la communauté (SEC) validés au niveau communautaire sont disponibles;
  • Les orientations techniques et opérationnelles susceptibles d’améliorer la mise en œuvre et les capacités de coordination du PNLP dans le cadre de PECADOM sont proposées ;
  • Un plan de travail et une stratégie permettant la mise en œuvre des activités de la PECADOM conformément au plan d’action de la proposition du Fonds mondial est fait ;
  • La quantification des besoins en intrants (CTA, TDR, MILD, Goutte Epaisse/frottis mince) au niveau communautaire est réalisée par le PNLP en collaboration avec son comité de quantification;
  • Un mécanisme efficace de travail avec les partenaires techniques et financiers, ainsi que ceux de la mise en œuvre de la prise en charge au niveau communautaire, est défini; 
  • Un système de coordination englobant les mécanismes de suivi évaluation, de rapportage du niveau communautaire vers les CSComs est proposé et validé; 

QUALIFICATIONS: 

  • Diplôme de Santé Publique ou équivalenteavec une expérience confirmée d’au moins 10 années dans l’appui à des Programmes de Lutte contre le Paludisme en Afrique
  • Une compétence en planification et gestion des programmes de santé (mise en œuvre et suivi) est requise ainsi que des capacités analytiques et de communication permettant de travailler avec les partenaires et de développer, expliquer et mettre en œuvre les approches et stratégies d’accélération des programmes pays
  • Une expérience de travail en équipe et de développement des mécanismes de travail entre personnes de nationalité et de culture différentes
  • Une expérience dans la mise en œuvre des politiques et stratégies de lutte contre le paludisme notamment la mise à échelles des CTA, TDR et des MILD est requise, ainsi que des capacités à transférer les compétences
  • Une connaissance avérée en informatique (Excel, Access, épi info) 
  • Une excellente connaissance du français parlé et écrit et une bonne maîtrise de la langue anglaise
  • Une expérience de travail avec les partenaires nationaux et internationaux et une bonne connaissance du partenariat « Faire Reculer le Paludisme » et des politiques sur le développement de mécanismes en appui aux pays seraient un atout.

Dans la première phase de la Série 6, il y avait deux récipients principaux (RP), dont le Programme National de Lutte contre le Paludisme (PNLP) rattaché au Ministère de la Santé. L’évaluation de la mise en œuvre de la Phase 1 de la subvention de la Série 6 Paludisme a permis de constater les insuffisances structurelles du PNLP, notamment en termes de programmation, de mise en œuvre, de suivi et de rapportage des résultats. Le système actuel de suivi-évaluation ne permet pas de rapporter les résultats programmatiques de manière fiable et dans les délais, à cause des difficultés dans la collecte de données de qualité à partir du premier échelon du système de santé. Suite aux problèmes identifiés par l’Inspecteur Général du Fonds mondial, PSI/Mali a été retenue comme RP et le PNLP comme sous récipiendaire  (SR). 

Pour adresser des difficultés que le PNLP a rencontrées durant la Phase 1, le Secrétariat du Fonds mondial a recommandé le recrutement d’un assistant technique, au niveau régional et sous - régional, pour appuyer les orientations du programme et améliorer ses capacités de coordination et de mise en œuvre.

Dans ce cadre, PSI/Mali recrute, au profit du PNLP, un Conseiller Technique Managérial pour l’assister dans la mise en œuvre de la subvention consolidée Paludisme du Fonds mondial pour une durée de deux ans.  Le conseiller technique reportera au Directeur du PNLP et sera basé dans les locaux du PNLP.

RESPONSIBILITES:

  • Concevoir et proposer le cadrage des orientations des activités du PNLP ;

  • Assister au suivi du plan d’action approuvé;
  • Appuyer le PNLP, en étroite coordination avec la PPM (Pharmacie Populaire du Mali) dans la quantification des besoins en intrants (CTA, TDR, Kits de paludisme grave, et MILD) ;
  • Proposer les moyens pour améliorer la prévision des besoins aux différents échelons de la mise en œuvre du programme ;
  • Assister a l’élaboration des plans détaillés d’approvisionnement et de distribution des CTA, Tests de Diagnostic Rapide (TDR), kits de paludisme grave, MILD et insecticides des différentes sources de financements acquises;
  • Définir  un mécanisme efficace de travail entre les différentes divisions du PNLP ;
  • Assister à l’élaboration et au suivi d’un plan de travail interne trimestriel au niveau du PNLP
  • Définir  un mécanisme efficace de travail entre le PNLP et les partenaires techniques et financiers dans la mise en œuvre des activités du plan stratégique nationale du PNLP; 
  • Appuyer le PNLP dans  la mise en place de la base de données (logiciel, production des outils de collecte et dotation des structures) ;
  • Former le personnel du PNLP sur la planification, l’exécution, le suivi et le rapportage des résultats (cadre de performance) ;
  • Assister le Direction du PNLP dans ses interactions avec les instances nationales du Ministère de la Santé, les Partenaires Techniques et Financiers du Programme, le Fonds mondial, et l’Agent Local de Fonds (LFA) qui assure la vérification des données relatives à la mise en œuvre des activités de la subvention ;
  • Accompagner la mise à échelle de la collecte électronique des données dans le cadre de la subvention, en prenant en compte les résultats de l’évaluation d’octobre 2013 ;
  • Assister à la mise en œuvre de toutes les recommandations de l’évaluation du PNLP par le RP ainsi que celles de l’atelier MESST
  • Fournir les avis techniques et l'expertise requis selon le besoin au PNLP et aux instances du Ministère de la Santé.

Position Overview: The M&E Specialist will support database and tool development under the supervision of the Research, Monitoring & Evaluation Manager. The M&E Unit supports the Programs teams in improving data collection, analysis, and learning activities to help ensure accountability and efficiency from start to finish for all programs and projects, both new and established throughout SFH. The Specialist may be assigned to focus on specific grants or projects that have the greatest need for database tools that can track service provision to beneficiaries over time as well as the location of clients by decimal GPS coordinates. Such databases must respond to all information requirements of the concerned program and should be able to generate useful and accurate reports.
Additionally, s/he may be asked to create other data collection and management tools as assigned by the Head- R, M&E Manager or the Director Health Services.

Responsibilities:
1) System Development
  • Assist M&E Manager in clarifying project information needs of Program staff based on observed and reported use of information
  • Support project/program staff on ways to properly document, organize and capture program progress
  • Identify strengths and weaknesses in existing data collection and management systems and propose solutions in cooperation with the M&E Manager
2) Data Management and Analysis
  • Construct Microsoft Access databases and other data collection tools and revise them under the supervision of the Head R, M&E Manager
  • Pilot tools in collaboration with Program staff and assess their compatibility with field conditions to ensure tools are as user friendly yet powerful as possible
  • Ensure that data queries are addressed in an accurate and timely manner
3) Capacity Building – Train Program and M&E staff in operation of drafted tools as necessary
4) Implementation of other new innovations that the volunteer feels are going to help the organization in terms of data management and safety
Required Skills and Experience:

  • University courses in information technology, computer science, data management or other relevant academic background

Technical Advisor, Local Capacity Development In Strategic Marketing & Sustainable Programming

OHMaSS is a network member of PSI, located in Port au Prince Haiti; registered and an active public health non-profit with a focus on reproductive health, HIV/AIDS prevention, malaria prevention and control, and maternal and child health since 1989 and achieving legal status in 1995.

Since 2010, PSI has implemented the USAID funded cooperative agreement, Support for International Family Planning Organizations (SIFPO) with an aim of strengthening PSI FP programs worldwide and increasing access to and use of FP.  Through USAID Haiti field support, PSI request SIFPO funds for the activities proposed in this document.  These funds will allow PSI to build on important and successful work to-date in family planning (FP) and maternal and child health (MCH), while addressing gaps in local capacity development, strategic marketing and distribution, and increasing access to essential services.

Additionally, support from KfW for the Caribbean Social Marketing Program (CARISMA) through the Caribbean Community Secretariat (CARICOM) to implement the HIV/AIDS Prevention and Sexual and Reproductive Health in the Caribbean (Phase IV) commenced in 2013 and will come to completion in 2015.  Support focuses on access to HIV/AIDS prevention services, regional monitoring and evaluation, sustainability planning, community outreach, product social marketing and support for sexual and reproductive health integration.

Prior to the earthquake of 2010, Haiti’s unmet need for contraception was already among the highest in the world at 38%, and one in four deaths among women aged 15-19 was due to pregnancy or child-birth related causes.[1]  Today, in 2013, the unmet need for family planning remains very high with 35%.[2]  Strengthening access to FP is of critical importance in Haiti to increase the coverage of birth spacing, decreasing the unmet need, and supporting women to fulfill their family planning choice.  Additionally, high rates of anemia (65% Child; 49% Women) and experiences of diarrhea in the two weeks preceding survey (21% among children under 5) exist.2 Also, the fourth leading overall cause of death in Haiti is diarrheal disease.[3] Continuation of WASH and ORS will also be essential in continuation of progress achieved to date.

Working nationwide, the goal of the proposed project is to improve the health status of the Haitian people,especially women and children.  To address these problems, current SIFPO and CARISMA programming builds on the successes founded during the PROMARK project.  Continued emphasis will be placed on increasing the use of effective health products, services, and behaviors in the areas of HIV prevention, family planning, and diarrheal disease prevention and treatment.  PSI and its partners will focus social marketing efforts on the significant drivers of behavior change and expand the reach of the current social marketing project into rural areas, while targeting those most at risk. 

In partnership with USAID, and in line with PSI’s “Locally Rooted, Globally Connected” strategy of building a tightly-linked global network of national member organizations with deep local roots and governance, PSI has put an intense focus on the capacity development of OHMaSS staff.  This LCD focus, with an emphasis on strategic marketing and distribution (marketing analytics, consumer insights and pricing processes), sustainability strategy and the Total Market Approach, will inform all project activities, and is the principal objective and task of the TA.


RESPONSIBILITIES: The Technical Advisor will come to Haiti and focus on the sustainable enterprise development of OHMASS, building local staff capacity in strategic marketing and distribution (including Delta for Distribution and the 4Ps); sustainability (SESI, new product launches, and a higher level strategic sustainability plan); the Total Market Approach (TMA); and log frame and proposal development.

  • Lead the capacity development and reinforcement of DELTA for Distribution development and implementation, including rapid design and implementation of research to better understand key barriers for non-users of our FP and WASH products, and to better segment the market.
  • Update marketing plans for all products focusing on Four P’s of marketing: Product, Price, Place, and Promotion of both FP and WASH products through a participatory process with in-country market assessment and stakeholder involvement, including donors, government, commercial and NGO partners.
  • Develop the baseline Total Market situation and use of the Total Market Approach (TMA) Calculator for each product, including a strategic total market plan analyzing trade-offs between utilization, equity, and subsidy.
  • Develop processes and procedures and oversee the implementation of market surveys to facilitate product launches, with two new social enterprise products to be launched. 
  • Develop a long term sustainability strategy plan to include a) self-financing from social enterprise product sales and franchise clinics; b) donor diversification including public-private partnerships; c) recognition by government of role of private sector and financing from the government (e.g. insurance system reimbursing services in our franchise clinics); d) cost efficiencies.  This includes a  strategic road map for OHMaSS product and service sustainability,
  • Building OHMASS program staff capacity to manage, track, and report on the social enterprise sustainability index (SESI) system in collaboration with regional efforts with the goal to significantly increase the index score Level 1.
  • support logframe/proposal development and efficient monitoring and evaluation systems as requested.

QUALIFICATIONS: <u
  • Relevant post-graduate degree (MPH, MSc, PhD, etc.);
  • 8+ years work experience in a developing country in a technical or management capacity;
  • Sound technical knowledge and implementation experience in complex setting in areas of social marketing and product distribution.
  • Experience with implementation of DELTA and DELTA for Distribution, social enterprise sustainability index, Women’s Health Project and/or SIFPO;
  • Established technical reputation in related areas and evidence of ability to navigate the Haiti partnership and private sector landscape.
  • Strong writing and presentation skills;
  • Familiarity with Haiti (or with countries with similar socio-economic and cultural profiles) and international donor/partner community;
  • Fluency in written and spoken French and English.

EMERGENCY RESPONSE: SUPPLY CHAIN COORDINATOR

In response to Typhoon Haiyan/Yolanda in the Philippines, Save the Children is in the process of putting together a team provide emergency response support. As part of this we are looking for a Supply Chain Coordinator to provide expertise and support to our upscale in the area.
